Bilingual Spanish/English Social Care Navigator – Remote

The Bilingual Spanish/English Social Care Navigator role is remote, with pay of $21–$23 per hour. Epicured’s New York Medicaid Section 1115 Waiver program supports Medicaid members through screening, navigation, and connection to eligible services. The navigator engages members, assesses eligibility for health-related social needs services, and coordinates referrals through Epicured and community partners. The role also tracks referrals, answers program questions, and maintains accurate records while following Medicaid, HIPAA, and New York State Department of Health requirements.

What you'll do

• Manage inbound and outbound member communications by phone and email using Five9 and partner platforms
• screen potential members and assess eligibility
• navigate members to service providers based on identified health-related social needs
• track and follow up on referrals
• maintain accurate documentation in program and internal systems
• support members with program questions
• comply with Medicaid, HIPAA, and New York State Department of Health requirements.
