What you'll do

• Recruit, onboard, train, coach, and manage community liaisons
• develop relationships with health systems, medical groups, payers, senior living communities, physician practices, and discharge teams
• oversee growth analytics and reporting
• track KPIs, pipeline health, and conversion
• analyze market and referral trends
• drive revenue targets
• coordinate with clinical operations, marketing, finance, quality, and compliance.
