Call Center Representative

The Call Center Representative role is onsite in Brentwood, TN. Starting pay is $21 per hour, or $22 per hour for candidates who meet the bilingual requirement. Monogram Health provides personalized, in-home care for people with complex chronic conditions and supports patients around the clock, seven days a week and on holidays. As an initial point of contact for patients, the representative conducts outreach, explains health-plan benefits and program services, and helps facilitate enrollment and appointment scheduling. Accurate documentation and privacy practices support patient access and the delivery of personalized care.

What you'll do

• Make outbound calls to prospective patients and receive inbound calls from patients and providers
• explain the value of Monogram Health and facilitate enrollment
• contact existing patients to schedule annual exams or re-engage them
• follow workflows and scripts to confirm HIPAA and health-plan eligibility
• protect patient privacy
• enter call data in Salesforce
• build rapport and trust
• use software and electronic health record platforms.
