What you'll do

• Build supportive relationships with youth and families
• coordinate service and discharge plans, progress reports, assessments, and service plan meetings
• maintain case management logs and program and medical files
• coordinate family visits, appointments, transportation, and aftercare
• attend FAPT meetings and court appearances as needed
• support staff training and supervision
• provide rotating administrator-on-call support
• maintain required certifications and training.

Why this role

Requires a qualifying master’s or bachelor’s degree in social work, psychology, or counseling, or a bachelor’s degree with specified experience and ability to obtain QMHP. Requires current CPR/First Aid, medication aide, behavioral management, and other listed training. Includes routine rotating administrator-on-call support.
