What you'll do

• Plan, organize, supervise, and direct Social Services Department operations
• oversee services and assessments intended to support residents’ well-being
• coordinate preadmission services and participate in interdisciplinary evaluations
• contribute to facility policy development
• oversee documentation, quality assurance, advance care planning, and review of advance directives
• support residents and representatives with care options and grievances
• and track grievance trends for the facility’s QAPI program.
