The Homecare Hospice CNA role is onsite in Asheboro, NC, with a Monday–Friday, 8 AM–5 PM schedule and travel to homes and facilities within 30 miles of the Asheboro office. Working under an RN Case Manager, the CNA provides personal care and home-management support in patients’ residences. The role helps meet care-plan goals, maintains a safe and comfortable environment, and communicates patient changes to nursing staff.

What you'll do

• Follow nurse aide care plans for hygiene, elimination, feeding, and mobility
• report patient condition changes and concerns to nursing staff
• participate in interdisciplinary conferences as needed
• document care
• coordinate visit timing with patients, families, and facility staff
• maintain a clean and safe environment
• reinforce home safety and sanitation
• provide light housekeeping and meal preparation
• average four to six visits per day.
