Case Manager, Residential Stabilization and Rehabilitation

The Case Manager, Residential Stabilization and Rehabilitation role is onsite in Hudson, NY; compensation is $22–$25 per hour. The position supports clients in a Stabilization/Rehabilitation residential setting. The case manager provides counseling and coordinates treatment-related support, monitoring client well-being and helping residents work toward their goals. The role contributes to safe, structured residential care through treatment planning, documentation, and coordination with program staff and emergency services when needed.

What you'll do

• Participate in client screening and admission information gathering
• observe clients’ physical, mental, and emotional status
• coordinate with the program manager on treatment needs and maintain a rotating caseload
• provide individual and group counseling
• collect urine drug screens and breathalyzer tests and complete related paperwork
• monitor safety and medication schedules
• prepare treatment and discharge plans
• manage and report incidents
• attend case conferences, resident meetings, staff meetings, and trainings
• contact emergency services as needed.
