Job overview

The Community Health Worker role is onsite on Maui, based in Wailuku, HI; remote work is not available. The stated pay is $25 per hour. The worker helps patients navigate and access health and community services through care-plan support, health education, referrals, and follow-up. The role works with advanced practice providers and helps address barriers to care using a culturally relevant approach.

What you'll do

• Support treatment goals and care plans created by advanced practice providers
• help patients improve healthcare access and health knowledge
• arrange community referrals and follow-up services
• collaborate on patient wellness and self-management goals
• respond to inquiries and assist with paperwork
• facilitate telephone or video visits
• educate patients about care processes
• identify barriers such as financial, transportation, insurance, and language needs and refer patients to resources.
