Patient Care Coordinator

This nonsupervisory role helps patients navigate referrals, screenings, and chronic-condition care by coordinating with providers, nursing staff, and the Referral Department and maintaining accurate follow-up information.

What you'll do

• Coordinate specialist referrals and diagnostic imaging orders
• follow up with patients about scheduling and access to care
• track outside records and results and document updates in the EMR
• partner with providers and nursing staff on care coordination
• maintain diabetes and hypertension registries and contact patients overdue for visits or screenings
• support population health, UDS reporting, PCMH activities, quality improvement, and PDSA work
• maintain confidential records in accordance with HIPAA and SPRHS policies.

Why this role
