The Registered Nurse (RN) Case Manager C3 role is based in Lynn, MA, with work from a federally qualified health center, home office, or enrollee homes as needed. Pay is $37.97–$51.10 per hour. Lynn Community Health Center's Complex Care Management team supports underserved people and families with complex or chronic conditions. The RN case manager assesses medical, behavioral, and social needs and works with enrollees and care teams to develop and coordinate care plans. The role helps address barriers, improve understanding of health conditions, and connect people with appropriate community resources.

What you'll do

• Meet enrollees in person or by phone
• conduct comprehensive assessments with the Complex Care Management team
• develop and implement care plans with enrollees, providers, and team members
• serve as a contact, advocate, and resource
• provide condition education and self-management support
• measure and maintain clinical, financial, and functional outcomes
• coordinate with behavioral health community partners and refer to community resources as appropriate.
