What you'll do

• Monitor patient situations and provide follow-up care
• develop therapy plans with goals and objectives
• assess mental health using medical information, interviews, observation, and testing, in collaboration with staff
• refer patients and family members to community resources or specialists
• provide crisis intervention as needed.
