What you'll do
- Ensure delivery of hospice services
- provide in-person patient care including assessments, admissions, and death visits
- conduct psychosocial and spiritual assessments
- support the Interdisciplinary Group (IDG) through chart reviews and care plan updates
- educate families on disease progression
- maintain clinical documentation
- assist with triage and field staff guidance.
