What you'll do

• Develop, lead, and facilitate team meetings
• provide care coordination in homes, communities, or preferred locations
• partner with youth, families, natural supports, and service providers
• maintain documentation for OhioRISE benefits
• maintain knowledge of community resources
• attend required trainings and supervision
• document services in databases and electronic health records
• travel within the assigned service area.
