What you'll do

• Provide outpatient primary care for patients of all ages

• perform preventive care, wellness visits, and screenings
• coordinate care with specialists and ancillary services
• use EPIC EMR
• participate in quality improvement and value-based care initiatives
• collaborate with Advanced Practice Providers and multidisciplinary teams.
