What you'll do

• Provide personal care according to individualized care plans
• maintain patient comfort and safety at home
• monitor and report condition changes to the RN Case Manager
• assist with transfers, mobility, and range-of-motion exercises
• support and educate families on care techniques
• document clinical notes
• and participate in team meetings and quality improvement initiatives.
