What you'll do

• Initiate admission assessments and initial care plans
• coordinate admission components with the interdisciplinary team
• audit records for compliance
• participate in discharge planning when needed
• review and evaluate care plans
• complete charting, MDS, and other assigned documents
• maintain resident confidentiality
• supervise or direct resident care when assigned.
