What you'll do

• Lead clinical operations of the Home Health department
• maintain compliance with Medicare Conditions of Participation and state regulations
• oversee clinical documentation and quality assurance
• review OASIS documentation and care plans
• support performance improvement
• develop workflows and best practices
• mentor clinicians
• collaborate on strategic planning
• partner with scheduling, billing, marketing, and administrative teams
• strengthen relationships with physicians, hospitals, and referral partners.
