Case Manager

Job overview

The Case Manager role is based in Lebanon, NH, with positions available throughout New Hampshire; compensation is up to $50,000 per year. Northeast Family Services provides mental health services to children, youth, and families, and this role reports to a Clinical Supervisor.

What you'll do

• Maintain an assigned caseload and implement in-home services based on DCYF referrals
• coordinate referrals, treatment plans, meetings, and service documentation
• provide parent education, coaching, and supervised visitation
• communicate with DCYF, courts, schools, behavioral health providers, and treatment team members
• assess client risk and consult the clinical supervisor about safety concerns
• transport clients
• and document clinical contacts. Early-evening availability and participation in an on-call rotation are required.
