What you'll do
- Independently provides care for patients with acute and chronic illnesses.
- Takes full accountability for patient care and outcomes.
- Engages with hospitalists for patient care.
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
- Leads care team.
- Engages with transitional care team for patients unable to come to the office.
- Supervises and collaborates with Advanced Practice Practitioners.
- Performs other duties as assigned.
What we're looking for
- Education
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
- Experience
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. Board Eligibility required.
