What you'll do

• Provide skilled nursing care under physician plans of treatment
• assess patients and home situations
• document changes and interventions
• complete OASIS assessments when required
• develop and update individualized care plans
• coordinate hospice services and participate in interdisciplinary assessments
• provide end-of-life care and patient/family education
• supervise licensed practical nurses and paraprofessionals
• maintain electronic records and communicate with physicians and care teams
• rotate weekday, weekend, and holiday on-call and backup-call duties
• travel to other offices as requested.
