Case Manager

The Case Manager role is onsite in Saint Louis, MO. Pay is $17.16–$20.28 per hour. Covenant House Missouri serves young people ages 16 to 24 who are runaway, trafficked, at risk, or experiencing homelessness, offering shelter, basic needs, and support toward independent lives. The case manager provides individualized, goal-oriented support and direct care to program youth through assessment, planning, advocacy, coordination, and trauma-informed interactions, helping youth work toward their individual goals.

What you'll do

• Build appropriate relationships with youth
• conduct intake and assessments
• identify needs and develop individualized treatment plans
• provide crisis intervention counseling to youth and families as needed
• collaborate with the interdisciplinary team
• advocate for youth and refer them to community services
• organize life-skills and group activities
• transport and accompany youth to appointments
• model positive behaviors and coping skills
• maintain records and documentation
• facilitate team and individual meetings
• administer medication and first aid according to agency policies
• assist with residential activities
• engage volunteers and partners
• complete required training and staff meetings.

Why this role

A bachelor’s degree in social work, human services, or a related field, or equivalent experience, is required; a master’s degree is preferred. Two years of experience is preferred. The role includes at least 40 hours of training annually.
