Bilingual Spanish Care Manager — Hybrid

What you'll do

• Complete intakes, assessments, reassessments, and care plans
• conduct home visits and community follow-ups
• participate in case conferences and supervisory meetings
• coordinate with clients, families, and providers
• monitor service use and medication adherence
• follow up after inpatient and ER discharge
• support appointments and preventive screenings
• assist with benefits, entitlements, and housing applications
• provide or arrange wellness education in the client’s preferred language
• assist with crisis intervention.
