The Hospice Registered Nurse (RN) Case Manager — Day Shift role is onsite in Scottdale, PA. The annual total cash target pay range is $79,600–$119,400. What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• refer patients to other hospice disciplines as needed or document why not
• maintain accurate and timely patient progress records
• record pain and symptom management changes and outcomes
• teach patients, families, and caregivers care techniques
• communicate regularly with the clinical manager and care team
• perform assigned on-call responsibilities and services.
