What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, implement, and document care plans
• educate patients and families
• determine service scope and frequency
• apply infection-control precautions
• provide clinical direction to hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes
• monitor cases for third-party payer compliance
• participate in in-service programs and QAPI
• perform assigned on-call responsibilities
• communicate with patients, families, team members, and healthcare professionals.

Why this role

PRN, paid per visit; current Registered Nurse license, driver’s license, and CPR certification required. Two years of clinical RN experience and one year of full-time hospice experience are preferred.
