What you'll do

• Complete Start of Care assessments and OASIS documentation
• perform skilled nursing assessments and visits
• develop and update individualized care plans
• monitor patient condition and communicate changes to physicians and the interdisciplinary team
• educate patients and families
• coordinate with therapists, physicians, caregivers, and other professionals
• maintain compliant clinical documentation
• travel throughout the Albuquerque/Rio Rancho service area.
