What you'll do

• Conduct initial screenings and referrals
• identify client needs and connect clients with community resources
• respond to emergencies and provide crisis assistance
• communicate with referring providers, particularly the state hospital system
• provide warm handoffs to partnering providers
• document services, referrals, and follow-up
• work with a multidisciplinary team and independently as needed.
