What you'll do
- Review referrals and assess eligibility
- complete intake and initial service assessments
- develop and monitor service plans and applicable monthly budgets
- maintain participant contact and required documentation
- help participants obtain community supports and services
- submit service logs and billing documentation
- attend required meetings and training
- respond to calls daily
- report significant concerns and critical incidents
- and rotate 24-hour on-call support.
