What you'll do

• Provide one-on-one skilled nursing care in patients’ homes
• follow individualized care plans
• assess patients and monitor changes
• administer medications and prescribed treatments
• provide assigned care such as G-tube, tracheostomy, ventilator, or seizure care when appropriate to the plan and verified competencies
• document care
• communicate with families and the clinical team
• provide patient and family education and support.
