Job overview
The Bilingual Field Care Coordinator is a field-based role in Orange County, CA, with four days a week of travel for patient engagement and one work-from-home day; the schedule is Monday–Friday, 9:00 a.m.–5:30 p.m. Pay is $30 per hour. Pair Team’s care model combines clinical, behavioral, and social support for high-need patients. The coordinator serves as a primary contact for members with complex needs, conducts in-person visits, and connects members with care and community resources. The role supports care continuity through individualized plans, follow-up, documentation, and collaboration with nurses and behavioral health care managers.
What you'll do
- Serve as primary contact for members with complex medical, behavioral, and social needs
- conduct in-person onboardings and visits at homes, in the community, and at partner organizations
- deliver care packages and collect health readings such as blood pressure
- coordinate services, appointments, and referrals
- collaborate with nurses and behavioral health care managers
- identify barriers to care
- maintain individualized care plans and timely documentation
- advocate for members
- participate in collaborative care meetings.

Why this role
Spanish fluency required. Requires at least one year of in-person field case management or care coordination experience, local familiarity, reliable transportation, a valid driver’s license, and current auto liability insurance. Gas and mileage are reimbursed; a $100 monthly work-from-home expense stipend is offered for WFH days.
