Hospital Liaison — Lafayette

What you'll do

• Maintain a daily presence at the primary account
• review and update CRM dashboards and forecasts
• respond to referrals through CarePort and other platforms
• provide definitive referral answers within an hour or by end of day when appropriate
• coordinate with business development and operations leadership to resolve delays and barriers
• update pending referrals with the AD Concierge
• coordinate with Transitional Advisors
• enter patient information into PCC
• prepare weekly reports and daily forecasts
• participate in quarterly business development meetings
• support guest relations and patient rounding.
