• Promote service lines and drive referral business
• follow up with patients during hospitalization and after discharge
• coach patients in developing personal healthcare plans
• assess patient needs
• facilitate support groups
• provide patient and professional education
• make welcome and follow-up calls
• collaborate with community teams and medical professionals.
