What you'll do

• Assess patients’ physical and psychosocial needs, develop and evaluate individualized care plans, provide nursing services within RN scope, triage patients, and handle patient calls and medication refill requests. Coordinate care across inpatient and outpatient settings, follow-up appointments, referrals, tests, and community resources
• educate patients and families, monitor progress, document in the electronic medical record, and participate in quality improvement.
