Home Health Registered Nurse (RN)

What you'll do

• Conduct start-of-care, recertification, resumption, and discharge visits
• complete OASIS assessments
• develop and update care plans with physicians and the care team
• provide wound care, IV therapy, medication management, and disease-management interventions
• teach patients and caregivers
• coordinate with therapy, aides, social work, and physicians
• document visits
• escalate changes in condition
• support quality and patient-satisfaction goals
• participate in case conferences and clinical education
• supervise home health aides and verify care plans
• contribute to quality improvement and efforts to avoid unnecessary readmissions.
