Community Health Worker

Job overview

The Community Health Worker role is onsite at the Clarion Community Health Center in Clarion, PA, Monday through Friday from 8:30 a.m. to 5:00 p.m. Starting pay is $18 per hour. Primary Health Network’s mission is to provide quality primary care and access to specialty care in the communities it serves. The role connects patients with healthcare and community resources, helps address barriers to care, and supports patients in navigating services. Outreach, education, advocacy, and care coordination contribute to patients’ health and well-being.

What you'll do

• Help patients identify and overcome barriers to healthcare
• connect patients with healthcare, social services, and community resources
• provide health education, coaching, social support, and advocacy
• assist with care coordination, appointments, transportation, insurance concerns, and system navigation
• conduct outreach and follow-up, including calls and home visits as appropriate
• document in the electronic health record.
