Support Planner

Job overview

The Support Planner role is hybrid in Waldorf, MD, with a schedule of 8:30 am–5:00 pm. Compensation is $55,000–$60,000 per year. Fresh Start Therapeutic Services provides person-centered case management for people enrolled in Medicaid Long-Term Services and Supports programs. The planner assesses participants’ needs, strengths, and preferences, develops and monitors individualized Plans of Service, and coordinates community-based services. Ongoing communication and documentation help ensure program compliance while supporting participants’ goals, independence, and quality of life.

What you'll do

• Assess applicants’ and participants’ needs, strengths, and preferences
• develop and monitor individualized Plans of Service
• coordinate community-based services
• maintain required documentation and program compliance
• communicate with participants, providers, and interdisciplinary team members
• conduct in-person visits and remote documentation within required timeframes.
