What you'll do

• Conduct member outreach, engagement, and enrollment
• support and update individualized care plans
• coordinate interdisciplinary care team discussions
• monitor progress and follow up on referrals, medications, appointments, and treatment plans
• assist with hospital discharge follow-ups and specialty care
• identify social needs and connect members with resources
• document interactions in the EHR
• comply with ECM requirements, clinic policies, and HIPAA.
