Certified Home Health Aide (CHHA)

Job overview

The Certified Home Health Aide (CHHA) role is based onsite at an adult day health center and primary care clinic in Oceanport, NJ; patient home visits and transportation may also be involved. Compensation is $17–$20 per hour. The role supports patients, including older adults, through an interdisciplinary care team. The aide helps implement individualized care plans and supports patients’ daily needs, safety, engagement, and well-being through personal care, monitoring, documentation, and collaboration with the care team.

What you'll do

• Assist with bathing, dressing, grooming, feeding, and toileting
• participate in individual and group activities
• assist with meals
• maintain a clean and safe environment
• record vital signs and weights and report health changes
• accompany patients to appointments and assist with transportation
• document care
• collaborate on individualized care plans
• protect patient confidentiality.
