The role supports Direct Support Professionals, helps implement individual support plans, coordinates household needs and schedules, and maintains timely progress documentation.

What you'll do

• Complete routine safety checks
• restock groceries and personal hygiene supplies
• assist with shopping trips, appointments, and budgeting
• train and support Direct Support Professionals in implementing Individual Support Plans
• create adequately staffed schedules
• model care and communication practices
• communicate with families
• complete online progress reports, goals, and daily notes.
