Rapid Rehousing Case Manager

The Rapid Rehousing Case Manager opportunity is onsite in West Haven, CT, with client hours Monday through Friday, 8:30 a.m. to 4:30 p.m. Compensation is not specified. The case manager helps people experiencing homelessness secure housing and supports their stability after move-in. The role coordinates housing applications, basic-needs resources, referrals, community connections, and employment assistance according to each client’s needs.

What you'll do

• Identify client needs and establish social-service support
• determine housing eligibility and assist with voucher applications
• help clients find and secure apartments
• screen documents
• assist with identification and applications for Medicaid, housing subsidies, food subsidies, and other entitlements
• initiate mental health and addiction treatment referrals as needed
• encourage social supports and community connections
• provide employment support
• continue case management after clients are housed.

Why this role
