What you'll do

• Perform nursing duties in accordance with the Nurse Practice Act and professional nursing principles
• provide information, assessment, and admission services to referred patients
• conduct assessment visits in homes, nursing facilities, or healthcare facilities as needed
• communicate with the Referral Department and physicians about referral appropriateness and certifications
• update the Referral Department on referral and admission progress
• provide nursing care, emotional support, and education to patients and families
• document in electronic medical records.
