What you'll do
- Screens telephone referrals and performs psychosocial evaluations.
- Develops and implements short-term service plans.
- Provides counseling, care management, and referrals for long-term treatment.
- Maintains case records and encourages resistant clients to accept services.
- Participates in interdisciplinary team meetings and provides outreach services in the community.
