What you'll do

- collaborate with physicians, nursing staff, and the interdisciplinary team on individualized care plans
- explain the admissions process, policies, and services to patients, families, and caregivers
- complete admission paperwork and obtain required records and forms
- coordinate assessments and services with hospice disciplines
- administer prescribed medications and treatments
- monitor vital signs and symptoms and report changes
- provide emotional support
- ensure compliance with admission regulations, documentation requirements, and patient rights
- participate in team meetings.
