Job overview

The Care & Unit Operations Coordinator role is on-site in Mahnomen, MN. Compensation is $23.50–$32.90 per hour. The position supports long-term care and other applicable services at Mahnomen Health Center.

What you'll do

• Coordinate medically related social services and admission, transfer, discharge, and care-transition planning
• complete or coordinate psychosocial assessments, care-plan documentation, and records
• support resident and patient rights, referrals, and staff education
• coordinate schedules across departments and maintain staffing, timekeeping, census, and administrative records
• order and monitor supplies
• participate in quality assurance and performance improvement activities and assigned audits
• communicate operational and care concerns to leadership.

Why this role

High school diploma or equivalent and at least one year of social services, case management, care coordination, or related healthcare experience are required. A relevant bachelor’s degree and Minnesota Social Work license are preferred when applicable.
