What you'll do

• Oversee patient care in the designated region, including assessments, staff supervision, care coordination, patient assignments, referrals, and individualized care plans. Lead clinical staff and monitor compliance and care delivery
• conduct weekly interdisciplinary team and leadership meetings
• develop care processes and policies
• assist with budgeting and census-related staffing adjustments
• oversee hiring, training, and performance reviews
• maintain patient records and supplies
• and support quality assurance and regulatory compliance.
