Direct Support Professional, Individual Community Living Supports (Full-Time)

Job overview

The Direct Support Professional, Individual Community Living Supports (Full-Time) role is onsite in Saint Paul, MN, with compensation of $19–$22 per hour. One Family Care supports people with disabilities and mental illness through advocacy, peer support, and independent living skills training. This role helps clients remain in their homes and build independence by providing personal care, household support, skills training, and assistance accessing community resources. The professional follows each client’s Community Supports and Services Plan and collaborates with the client’s support team.

What you'll do

• Follow Community Supports and Services Plans
• collaborate with clients’ support team members
• help clients set goals and develop plans
• assist with mobility, personal care, hygiene, meals, and light housekeeping
• drive clients to outings, stores, and appointments
• train clients in household care and skills such as cooking, budgeting, paperwork, and appointment management
• help clients access community resources
• assess and mitigate risks
• support activities of daily living and implement behavior-management strategies.
