Enrollment Specialist

The Enrollment Specialist role is onsite in Pittsboro, NC, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. Compensation is $22.49–$30.23 per hour. The role supports Piedmont Health Senior Care’s PACE program, which helps seniors maintain independence and continue living at home through coordinated, personalized care. The specialist manages intake and enrollment activities, evaluates prospective participants’ needs and eligibility, and coordinates applications and assessments with families, care teams, and public agencies. The role also supports outreach and referral relationships so eligible seniors can access the program and applicants who are not accepted can be connected with other community resources.

What you'll do

• Evaluate prospective participants’ needs and eligibility
• conduct assessments in participants’ homes as needed
• coordinate Medicaid level-of-care and Medicare applications with relevant agencies
• participate in team meetings and outreach
• document assessments and maintain statistical data
• present intake reports
• travel to homes and community agencies as needed
• help facilitate care plan and enrollment agreement execution
• provide alternative community resources to applicants not accepted
• maintain relationships with referral sources.
