Hospice Nurse Liaison

The Hospice Nurse Liaison is an onsite, full-time, 40-hour-per-week day-shift position in Norwich, CT. Pay is $38–$49 per hour. The role supports Masonicare Home Health & Hospice and its hospice admissions and referral services. The liaison evaluates patients for hospice services, completes admissions and assessments, develops initial care plans, and coordinates referrals. The role also educates patients and families, manages assigned cases, and builds referral relationships to support appropriate transitions into hospice care.

What you'll do

• Assess patients for hospice appropriateness
• educate patients and families and obtain admission consents
• complete GIP and routine hospice admissions, HOPE and comprehensive assessments, and documentation
• coordinate referrals and DME orders
• develop patient-specific care plans
• reconcile medications and provide teaching
• manage assigned caseloads and discharge planning
• support referral contacts and relationships.
