Home Health Registered Nurse (RN) Case Manager

Job overview

The Home Health Registered Nurse (RN) Case Manager role is onsite in Grand Rapids, MI, providing care in patients’ homes, and pays $60–$70 per hour. What you'll do

• Provide skilled nursing services
• initiate preventive and rehabilitative nursing procedures
• prepare clinical and progress notes
• coordinate patient services
• notify physicians and staff of changes
• complete medical history, home safety, and environmental assessments
• develop individualized plans of care for physician approval
• determine medical necessity for other services
• reevaluate nursing needs and activities of daily living
• develop and implement home health aide plans when ordered and supervise aides
• coordinate other professional and community services
• communicate with patients, families, and the care team
• review billing processes with patients or families.
