What you'll do

• Document care according to organization policy
• report deviations from the Plan of Care to the Case Manager
• use durable medical equipment safely
• take vital signs when included in the Hospice Aide Plan of Care
• participate in team and staff conferences
• observe skin and report concerns
• assist with ambulation, transfers, and range of motion when documented in the plan
• provide respite for family or caregivers when appropriate
• participate in QAPI activities and IDG meetings as appropriate.
