What you'll do

• Conduct initial psychosocial, emotional, spiritual, and bereavement assessments
• help develop and implement goal-directed care plans
• participate in interdisciplinary team conferences
• document visits, calls, and referrals
• reassess needs and provide counseling
• provide crisis intervention when indicated
• advocate for patients and families
• coordinate community referrals
• provide counseling related to death and dying
• participate in the Bereavement Team as assigned.
